When You Arrive
If you have been issued a Med-Key card, please bring your card along with your insurance card and present them upon arrival. If you do not have a Med-Key card, or you have not already been pre-registered with our facility, we ask that you bring the following information with you in order to expedite the admission process:
- A picture ID, such as a driver’s license
- Your insurance card
- Your contact information (addresses and phone numbers)
- Your identifying information (date of birth and social security number)
- Emergency contact information
- Medical history, including current medications
- Physician orders, if given to you
- Advance Directives, if you have them prepared. If you wish to prepare Advance Directives you can print out the form on the main Patients page and bring the forms with you
